About The Position

We’re seeking a bold, compassionate Family Nurse Practitioner (FNP) to join a fast -paced, patient -first Direct Primary Care (DPC) team in Metro Detroit. This role is ideal for a provider who values relationship -based medicine and is committed to delivering high -quality care to a diverse community.

Requirements

• Active Michigan Family Nurse Practitioner (FNP) license
• Master’s Degree in Nursing or related field
• Bilingual in English and Spanish
• Strong clinical judgment, patient advocacy mindset, and commitment to relationship -based care

Nice To Haves

• 2–5 years of experience in Primary Care
• Experience in outpatient, community -based, or value -based care models
• Comfort working in a fast -paced but patient -focused clinical environment

Responsibilities

• Provide comprehensive primary care to patients across the lifespan
• Deliver culturally responsive, patient -centered care
• Assess, diagnose, treat, and prescribe medications as appropriate
• Educate patients and families on prevention, wellness, and chronic disease management
• Document care accurately and efficiently using established workflows
• Collaborate with clinical and administrative teams to support continuity of care
• Support patient communication in English and Spanish
