About The Position

Our patient-centered team is dedicated to improving health outcomes for diverse and underserved communities. If you’re passionate about health equity and ready to bring whole-person care to those who need it most, we’d love to meet you. We are seeking a Full-time Family Nurse Practitioner to deliver comprehensive, outpatient care. You’ll also participate in health education and community outreach efforts that promote wellness and increase access to care. This position requires availability on the first and last Thursday and Saturday of each month, from 10:00 AM to 7:00 PM.

Requirements

• Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited program
• Certified as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)
• Active and unrestricted NYS Nurse Practitioner license and current DEA registration
• BLS certification and infection control certification required
• Commitment to working with diverse and underserved communities

Responsibilities

• Provide holistic medical care to patients in a community health setting
• Diagnose and manage acute and chronic conditions in collaboration with a multidisciplinary team
• Promote preventive health through screenings, education, and outreach
• Maintain accurate and timely electronic medical records (EMR)
• Contribute to health education and patient engagement initiatives
• Foster an inclusive and welcoming care environment for all patients
