About The Position

Wallace is hiring a Primary Care Family Nurse Practitioner (FNP) to join their team in Portland, OR to serve as an empaneled PCP, providing full scope, longitudinal primary care for patients across the lifespan in a supportive, team-based clinic environment. Requirements

Current, unrestricted Nurse Practitioner license in Oregon
Family Nurse Practitioner (FNP) certification (role serves pediatrics through geriatrics)
Current DEA and BLS
Commitment to team‑based, patient‑centered primary care

Nice To Haves

Experience in outpatient primary care, community health, or FQHC settings
Comfort managing chronic conditions and preventive care across the lifespan
Experience serving Medicaid or higher acuity populations
Collaborative, organized, and patient‑focused clinical approach

Responsibilities

Serve as an empaneled Primary Care Provider, delivering comprehensive care to assigned patients
Provide preventive care, chronic disease management, and episodic visits
Conduct patient histories, physical exams, and ongoing clinical assessments
Diagnose, treat, and manage acute and chronic conditions
Order, interpret, and act on labs and diagnostic testing
Prescribe medications and develop longitudinal treatment plans
Provide patient education, counseling, and care planning
Collaborate closely with behavioral health, nursing, and care coordination teams
Maintain timely, accurate documentation and population health follow‑up
Participate in quality improvement and team-based care initiatives
