Requirements

  • Licensed Independent Social Worker
  • Experience working in behavioral health, social work, or mental health services preferably in a home setting
  • Prior experience in clinical case review or utilization management
  • Familiarity with electronic health records (EHRs) and documentation systems
  • Experience with Medicaid-funded programs or billing workflows
  • Strong client engagement and rapport-building with willingness to learn from families and respect diverse values and experiences.
  • Assess family dynamics and respond to crises with sound, real-time decisions.
  • Ability to interpret mental health assessments and ensure documentation meets Medicaid standards.
  • Skilled in reviewing clinical records for accuracy, completeness, and billing eligibility.
  • Capable of identifying gaps and coordinating with providers to ensure compliant documentation.
  • Knowledge of community resources and child/adult development
  • Effective oral/written communication and organization
  • Ability to work independently and meet deadlines
  • Receptive to feedback and adaptable
  • Comfortable with teleconferencing tools
  • Excellent computer proficiency (MS Office - Word, Excel and Outlook)
  • Flexible schedule: daily cell phone use and travel
  • CPR and First Aid certification (current or obtainable)
  • Understanding of confidentiality standards
  • Valid driver’s license, reliable transportation, and insurance
  • Must pass criminal background check per service standards

Responsibilities

  • Deliver structured, goal-oriented FCT in clients’ natural environments.
  • Conduct crisis intake, assessments, interventions, and aftercare planning.
  • Develop measurable family goals and monitor progress.
  • Maintain fidelity to the FCT model, including documentation and court testimony.
  • Provide a minimum of two multi-hour face-to-face sessions weekly per family.
  • Work flexible hours, including evenings/weekends, to meet family needs.
  • Facilitate team meetings and case presentations using MIGS format.
  • Supervise staff pursuing prevention certification (RA, OCPSA, OCPS)
  • Ensure ethical practice, integration of training, and compliance with education and experience requirements.
  • Review and validate external mental health assessments to ensure they meet Medicaid documentation standards, including diagnostic clarity, service justification, and alignment with billing requirements for Family Centered Treatment program.
  • Supervisory duties for staff as they progress in the 4 stages of FCT
  • Oversight of FCT cases
  • Maintain accurate , timely documentation per agency and COA standards.
  • Conduct assessments using outcome tools.
  • Engage with stakeholders to support client progress.
  • Ensure 50% of work hours are in direct contact with clients.
  • Complete Wheels of Change training within 8 weeks of hire.
  • Achieve FCT certification (online and field-based) within 6–9 months.
  • Maintain certification through ongoing training and recertification every 2 years.
