SUMMARY Provides service and develops treatment plans that are based on a person-centered assessment of the individuals strengths, needs and is goal oriented . Provides 24/7 on-call support for all assigned individuals. Builds and maintains positive relationships with community partners from referring or collateral agencies.

Requirements

  • Bachelor’s degree (or masters in some states) in Social Work, Mental Health Counseling, and Clinical Psychology, Counseling Psychology or other related field
  • One or more years related field experience, two years preferred
  • Specialized training required
  • All counselors must hold a master's degree in counseling education, counseling psychology, or rehabilitation counseling from an accredited educational institution
  • Must have had two years of full-time supervised clinical experience in a multidisciplinary mental-health setting subsequent to obtaining the master's degree.
  • Must hold a Bachelor’s (Master’s preferred) degree in social work or Master’s degree in counseling from accredited educational institution
  • Must hold a license in: Licensed Social Work (LSW), Licensed Practicing Counselor (LPC) or Marriage and Family Therapist (MFT) required
  • Driver’s License and acceptable driving record

Responsibilities

  • Provides person-centered assessments in the areas of behavioral health, education, personality, community engagement, vocational needs, relationships, and family interactions.
  • Develops and revises service or treatment plans based on assessment of strengths, needs, collaboration with referral agencies, collaboration with the individual served and is goal oriented.
  • Provides activity and participation based interventions and approaches developed from training materials, experience, individual and/or families’ successes.
  • Provides 24/7 coverage for all assigned individuals served and/or families.
  • Makes referrals to services and resources on behalf of individuals served and/or families.
  • Assures that documentation and records are kept confidential, organized, and complete and submitted to supervisor in an accurate and timely manner.
  • Participates in team meetings with peers and in collateral team meeting when needed. Supports peer development by addressing areas of concern for professional development and team performance issues.
  • Provides a discharge and aftercare plan for each family/individual served.
  • Builds and maintains positive relationships with community partners from referring/ collateral agencies. Provides guidance for the family in their interaction with referring agencies, and other community organizations as needed.
  • May provide training or field supervision of new or tenured peers; review and edit peer records and documentation, marketing and education presentations; training and development for the region or team meetings
  • Performs other related duties and activities as required
