For 35+ years, Mentor South Bay has delivered Early Intervention, ABA, Behavioral Health, and Outpatient Mental Health services across MA and CT. What you’ll do Complete strengths-based assessments (mental status, behavioral patterns, relationships, family interactions) and, where applicable, diagnostic formulation. Develop individualized treatment plans. Provide in-home and community-based interventions; partner closely with the Family-Centered Support Specialist (FCSS) to implement plans. Coordinate care with schools, community partners, and referring/collateral agencies; connect families to resources. Ensure timely, accurate documentation and support team learning through case consultation and meetings. Provide clear closure and aftercare planning. Attending meetings; including team and supervision
