About The Position

The Family Case Aide at Brooklyn Catchment SBES supports children and families by assisting with service plans that promote safety, stability, and well-being. The role includes conducting home visits, supervising visits, connecting families to resources, and documenting services. The aide works closely with case planners to strengthen family functioning and help prevent foster care placement.

Requirements

  • One year of relevant work in child welfare or social services or life experience; OR Successful completion of an Administration for Children’s Services-funded prevention services program or similar program; OR A high school diploma or equivalent.
  • Good interpersonal skills, ability to work with diverse social, economic, and cultural groups.
  • Proficiency in Microsoft Office.
  • Ability to plan and carry out assignments independently.
  • Ability to prioritize, adhere to timelines and multi-task.

Responsibilities

  • Assist Family Coordinators in conducting assessments, developing service plans, and monitoring progress for families enrolled in the prevention program.
  • Provide logistical support for case-related activities, such as scheduling appointments, coordinating transportation, and arranging childcare.
  • Assist in facilitating parent/caregiver support groups and workshops, providing logistical support and assistance as needed.
  • Help families navigate community resources and access services related to housing, healthcare, education, and other basic needs.
  • Maintain accurate and up-to-date case records, progress notes, and documentation in accordance with program requirements and confidentiality standards.
  • Assist with data entry, report preparation, and other administrative tasks to support program evaluation and reporting efforts.
  • Work collaboratively with Family Coordinators, school staff, community partners, and other stakeholders to ensure coordinated service delivery and support for families.
  • Communicate regularly with Family Coordinators to provide updates on client needs, progress, and any challenges encountered.
  • Maintain regular consistent and professional attendance, punctuality, personal appearance, and adherence to relevant health & safety procedures.
  • Safe and successful performance, including meeting productivity standards.
  • Maintain an understanding of the principles, methods and processes needed to perform the job. Attends staff meetings, seminars and in-service training as assigned.
  • Demonstrates the ability to complete work in an acceptable manner by the time and/or date established with accuracy, neatness and thoroughness and minimal number of errors.
  • Maintain established departmental policies and procedures including maintain strict confidentiality and adherence to all HIPAA guidelines at all times.
