Requirements

  • Current licensure as a Social Worker (LSW, LISW) or Registered Nurse (RN) in the State of Ohio and at least one year experience in home care, geriatrics, home health care, medical social work or providing information and assistance or information and referral;
  • Basic knowledge of major chronic illnesses, disease processes and normal aging
  • Basic knowledge of medical terminology;
  • Excellent verbal and written interviewing and assessment skills;
  • Ability to determine if caller is experiencing a crisis or other emergency situations and be able to make an immediate referral to the appropriate emergency responders;
  • Knowledge of local resources for older adults and adults with disabilities;
  • Ability to record information in an accurate, timely manner is essential;
  • Ability to communicate in a collaborative, effective manner with others and to maintain good working relationships;
  • Basic computer skills, including Microsoft Office applications, the Ohio Department of Aging A&D/SAMS reporting system; and
  • Valid Ohio Driver’s License, proof of liability coverage, and a car available during work days.

Nice To Haves

  • Prefer an individual who is an Alliance of Information and Referral Systems (AIRS) Certified Information Specialist or one who is willing to pursue AIRS Certification as a WRAAA employee;

Responsibilities

  • Conducts telephone or in-person assessments to evaluate needs of family caregivers.
  • Assists family caregivers in developing a care plan, coordinates services and monitors the plan for effectiveness
  • Coordinates reimbursement to eligible caregivers
  • Obtains information during assessment to determine need for referrals and assists with accessing long-term care and other services and supports. This also includes any agency project or contract programs in which the agency is participating
  • Provides information, assistance and referrals for services to grandparents raising grandchildren
  • Advocates for callers who require extra support to access necessary services
  • Makes immediate referrals to emergency responders when the caller is in a crisis or emergency situation; conducts follow-up as appropriate to make sure that vulnerable individuals in difficult situations received assistance
  • Maintains consumer confidentiality according to agency policy
  • Reports abuse, neglect or exploitation of older adults as a mandated reporter as required by Ohio law.
  • If licensed as a Social Worker (LSW, LISW) or Registered Nurse (RN) abides by the professional code of ethics established by licensure.
  • Participates in trainings, meetings, in-services and continuing education opportunities.
  • Meets time frames for timely and accurate completion of documentation, including use of the A&D/SAMS reporting system
  • Will be required to travel to community and consumer locations within our five county service area (Cuyahoga, Geauga, Lake, Lorain and Medina counties)
  • Will be required to travel outside our PAA service area, including travel to Columbus, Ohio
  • Represents WRAAA at community events and caregiver presentations
  • Performs other duties as assigned by supervisor as needed to meet agency needs.
  • All staff have the responsibility and authority to propose ways to improve quality and to fully participate in quality improvement efforts
