Requirements

  • High school diploma or equivalent + 3 yrs experience in social services OR a ssociates degree or higher in human services or related area.
  • Bilingual Spanish/English.
  • Strong communication and writing skills.
  • Experience working with culturally and linguistically diverse populations.
  • Attention to detail with the ability to perform assignments and job responsibilities efficiently, accurately, and on time.
  • Proficiency in Microsoft Office and Windows
  • Valid US state driver’s license
  • Possess an insured & reliable vehicle and a valid AZ driver’s license.
  • Valid Arizona Fingerprint Clearance Card - OR - must qualify for a valid Arizona Level One Fingerprint Clearance Card
  • DES Criminal Affidavit Required
  • AZ DHS Criminal History Affidavit Required
  • FPC DPS Verification Required

  • Knowledge of family dynamics and community resources is preferred.

Responsibilities

  • Establish trusting relationships with families, in consideration of families’ personal experiences and diverse cultural backgrounds.
  • Meet with families in the hospital to engage families and gain referrals into the program.
  • Enroll families into the Healthy Families program.
  • Promote community awareness regarding Healthy Families program services.
  • Develop positive relationships with community providers to secure referrals and provide support to families.
  • Engage in face-to-face contact with community partners to provide presentations.
  • Compose accurate and complete in-depth assessments, program intake documents, and data tracking regarding the status of screening/assessment activity, within appropriate timelines.
  • Provide comprehensive data, recording-keeping and reporting of community outreach efforts.
