Requirements

High school diploma or equivalent + 3 yrs experience in social services OR a ssociates degree or higher in human services or related area.
Bilingual Spanish/English.
Strong communication and writing skills.
Experience working with culturally and linguistically diverse populations.
Attention to detail with the ability to perform assignments and job responsibilities efficiently, accurately, and on time.
Proficiency in Microsoft Office and Windows
Valid US state driver’s license
Possess an insured & reliable vehicle and a valid AZ driver’s license.
Valid Arizona Fingerprint Clearance Card - OR - must qualify for a valid Arizona Level One Fingerprint Clearance Card
DES Criminal Affidavit Required
AZ DHS Criminal History Affidavit Required
FPC DPS Verification Required

Responsibilities

Establish trusting relationships with families, in consideration of families’ personal experiences and diverse cultural backgrounds.
Meet with families in the hospital to engage families and gain referrals into the program.
Enroll families into the Healthy Families program.
Promote community awareness regarding Healthy Families program services.
Develop positive relationships with community providers to secure referrals and provide support to families.
Engage in face-to-face contact with community partners to provide presentations.
Compose accurate and complete in-depth assessments, program intake documents, and data tracking regarding the status of screening/assessment activity, within appropriate timelines.
Provide comprehensive data, recording-keeping and reporting of community outreach efforts.
