About The Position

To provide comprehensive primary care services to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic conditions. The Family APRN will work collaboratively within a healthcare team to deliver high-quality, patient-centered care. Cumberland Family Medical Center (CFMC) operates a Federally Qualified Health Center (FQHC) offering a wide range of health and social services at multiple sites throughout Kentucky. Requirements

  • Experience working with diverse patient populations.
  • Experience with electronic health records (EHR) systems.
  • Comprehensive knowledge of family medicine principles, practices, and procedures.
  • Ability to assess, diagnose, and treat patients across lifespan.
  • Excellent communication, interpersonal, and patient education skills.
  • Strong clinical decision-making and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Proficiency in performing physical examinations and ordering appropriate diagnostic tests.
  • Ability to manage chronic conditions and provide preventive care.
  • Knowledge of current healthcare regulations and guidelines.
  • Cultural sensitivity and awareness.
  • Commitment to providing high-quality, patient-centered care.
  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
  • Current and unrestricted Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) licenses in the state of Kentucky (or eligibility for licensure)
  • National certification as a Family Nurse Practitioner (FNP)
  • Full-time position, 4 days per week, 100% clinic-based, in person work required.
  • Work schedules are set at the discretion of CFMC and may be adjusted at any time based on the operational needs of the organization.

Responsibilities

  • Obtain comprehensive medical histories and conduct physical examinations.
  • Diagnose and treat common illnesses and injuries.
  • Order and interpret diagnostic tests (e.g., lab work, imaging studies).
  • Prescribe medications and other treatments as needed.
  • Develop and implement individualized care plans.
  • Counsel patients on preventive health measures, such as vaccinations and healthy lifestyle choices.
  • Provide education to patients and their families regarding their medical conditions and treatment plans.
  • Make referrals to specialists and other healthcare providers as appropriate.
  • Collaborate with other members of the healthcare team, including physicians, nurses, and other support staff.
  • Maintain accurate and up-to-date patient records.
  • Stay current with the latest medical advancements and best practices.
  • Participate in quality improvement initiatives.
  • Adhere to all applicable regulations and guidelines.
  • Demonstrate a commitment to providing compassionate, patient-centered care.
  • Other duties as assigned.
