About The Position

The PuroPREP PLUS program equips youth with the relational knowledge, skills, and tools to help them successfully transition to adulthood by implementing interventions that result in positive effects such as socio-emotional skills, prevention of risky behaviors, development of healthy relationships, among others. These services are offered in Harris County, Texas in partnership with community faith-based organizations The ideal candidate for our program is a self-motivated and dedicated person who is excited and passionate about helping youth learn ways to build healthy relationship skills for now and the future while supporting positive socioemotional development and successful transition to adulthood. You are also a personable, energetic, and empathetic leader who can support staff and projects in a prioritized manner to meet our internal Urban Strategies goals of PuroPREP PLUS Standards. Requirements

  • Bachelor’s degree in social work, Human Services, Sociology, or other behavioral science fields, master's preferred.
  • 3years of experience in case management.
  • Knowledge of family preservation, kinship care, informed care, child welfare policies and procedures, and relevant state and federal laws and regulations
  • Fluent in English and Spanish (verbal/written).
  • Effective communication and interpersonal skills to successfully interact with families, caseworkers, and other stakeholders.
  • Strong organizational and problem-solving skills to manage multiple tasks and priorities.
  • Ability to handle sensitive and confidential information with discretion.
  • Proficiency in Microsoft Office and data entry systems.
  • Must have Valid Driver’s License, Personal Vehicle, and Liability Insurance required.

Responsibilities

  • Oversee all Comprehensive Case Management activities within the program and provide direct supervision to the Family Advocate & Community Liaison.
  • Process intakes, assess participant needs, coordinate referrals, and ensure follow-up to support access to appropriate services.
  • Manage a caseload of assigned participants; intakes, assessing participant needs, coordinating referrals, and ensuring follow-up to support access to services.
  • Develop Individual Development Plans (IDPs) to meet the unique needs of youth, offering guidance that reflects their distinct roles and circumstances.
  • Screen all participants for Domestic Violence at intake.
  • Track participant progress and outcomes.
  • Maintain regular contact with assigned youth.
  • Coordinate services (financial, education, parenting, and DV support).
  • Document services accurately and on time.
  • Follow mandated reporting requirements.
  • Maintain strict confidentiality.
  • Communicate regularly with team and Program Manager.
  • Escalate safety concerns immediately.
  • Support workshop retention and completion.
  • Facilitate effective communication with other departments and stakeholders.
  • Maintain accurate records, files, forms, statistics, and additional relevant information in accordance with agency policy, licensing, and/or funding requirements.
  • Coordinate with third party contractors and other outside service providers to share and staff all pertinent information and updates on cases and participate in weekly case staffing.
  • Enter, upload, and submit accurate documentation in case management database.
  • Adhere to all timeframes and deadlines as per policies and procedures.
  • Attend external and internal program training and meetings when required.
  • Participate in activities or supplemental training that promote professional growth and development.
  • Able to react to change productively, fast-paced environment, and handle other essential tasks as assigned.
  • Flexible availability during evenings, weekend rotation. and on call as needed to assist with program emergencies, crisis, requested by supervisor per implementation schedule.
  • Employer retains the right to change or assign other duties to this position.
