About The Position

Responsible for connecting families with social, medical, and mental health services. Assesses client needs, develops, implements and reviews service plans, working closely with Family Navigators, Nurse Navigators, and other community resources in meeting/achieving client service needs. Works closely with First 1000 Days team to carry-out the mission and vision of the First 1000 Days initiative. Weekend and evening hours will be required, as well as attending community events. Requires travel.

Requirements

• Bachelor Degree
• Basic Life Support/CPR

Nice To Haves

• Prefer one (1) year of experience working with pregnant women/new parents and children under the age of three (3).
• Prefer proficiency in using Microsoft Office software, as well as remote conferencing software.
• Prefer knowledge of Sarasota, Manatee, and/or Desoto County services.
• Prefer bilingual (Spanish-speaking) fluency.
