About The Position

The Illinois Department of Public Health is seeking a highly motivated, detailed orientated individual to receive and review facility reported incidents for long term care (LTC) facilities, Intermediate Care Facilities for individuals who are intellectually disabled (ICF/IID), Specialized Mental Health Rehabilitation Facilities (SMHRF), and Assisted Living (AL) establishments. This individual will triage the Facility Reported Incidents (FRIs) and enter into the ACTS database timely. Actively participate in the planning, development and implementation of policies and procedures under the Special Investigations Unit.

Requirements

  • Requires (a) graduation from an approved nursing education program resulting in an associate or a diploma degree in nursing and three (3) years of professional nursing experience or, (b) a bachelor’s degree in nursing and two (2) years of professional nursing experience or, (c) a master’s degree in nursing.
  • Requires licensure as a Registered Nurse in the State of Illinois.
  • Requires ability to travel.
  • Requires valid and current Driver's license.
  • Requires ability to meet all agency vaccine/health-related polices and guidance.

Nice To Haves

  • Certified federal long term care surveyor, having passed Modules A and B of the Federal Surveyor Minimum Qualification Test (SMQT).
  • Elementary knowledge in conducting surveys in skilled and intermediate care nursing home facilities and shelter care facilities.
  • One (1) year experience conducting survey activities in nursing home and related facilities.
  • One (1) year experience with federal and state rules and regulations concerning licensure and certification of long-term care facilities.
  • Elementary knowledge of the on-site process of monitoring visits to non-compliant facilities.
  • One (1) year experience in nursing principles and practices.
  • One (1) year experience preparing accurate reports of survey findings.
  • Ability to develop and maintain harmonious and productive working relationships with various administrative and medical personnel, local governmental agency personnel and recipients of health care program.

Responsibilities

  • Receives facility reported incidents for long term care (LTC) facilities, Intermediate Care Facilities for individuals who are intellectually disabled (ICF/IID), Specialized Mental Health Rehabilitation Facilities (SMHRF), and Assisted Living (AL) to determine, based on licensure and state and federal regulations, if a complaint will be entered [into ACTS] upon the initial Facility Reported Incident report or on the final Facility Reported Incident FRI) Reports.
  • Enters facility reported incidents on Smartsheet’s or designated logging and tracking database daily to ensure accuracy of program daily, weekly, monthly, quarterly, and annual reports.
  • Collaborates with the Special Investigation Unit (SIU) Abuse, Neglect, Theft Coordinator, Abuse Prevention Review Team Manager, and the Monitor Program Manager regarding facility reported incidents (FRI).
  • Travels to attend staff meetings, inter-agency meetings and workshops as a means of maintaining technical expertise in the field of long-term care.
  • Performs other duties as required or assigned within the scope of the responsibilities enumerated above.
