About The Position

Gateway Rehab Center (GRC) has an opening for an LPN or RN Extended Care Nurse in Center Township, PA. In this role, you will provide assessment, nursing diagnosis/treatment planning, and direct nursing care to GRC patients. This position receives supervision and instruction from the Executive Regional Director of Nursing.

Requirements

• Pennsylvania or Compact RN/LPN unrestricted license.
• One year nursing experience, addiction medicine and med surg nursing experience preferred.
• Active CPR certified required.
• PA Criminal Background Check
• Obtain a PA Child Abuse and FBI Fingerprinting Clearances
• Pass Drug Screen
• 2-Step TB Test

Nice To Haves

• One year nursing experience, addiction medicine and med surg nursing experience preferred.

Responsibilities

• Conducts patient triage, assessments, reassessments, medical interventions, phlebotomy & POC testing as needed.
• Complete audits for the extended care programs as required.
• Interacts with the physician through coordination and the provision of assistance via in person, telephone, med wand or other platforms.
• Collaborates with pharmacy on medication issues.
• Coordinates with all GRC departments as appropriate and care coordination with outside agencies as needed.
• Attends GRC mandatory in-services and trainings.
