About The Position

A Clinical Pharmacist II provides comprehensive pharmaceutical care primarily through a decentralized model providing service excellence, clinical services, distributive functions, education, and a commitment to the department’s mission. He/she shall demonstrate leadership qualities, serve as a role model, and actively participate in development and revision of policies and procedures, evidence based protocols/order sets, and quality/performance improvement projects. Clinical Pharmacists II shall possess characteristics to successfully fulfill their responsibilities, including effective communication, flexibility, attention to detail, initiative, cooperation, and a high level of knowledge and skills applicable to their area of practice. Emphasis in practice area may include Cardiology, Critical Care, Emergency Medicine, Infectious Disease/Antimicrobial Stewardship, Medication Safety, Neonatology/Pediatrics, and/or Surgery.

Requirements

  • Bachelors Degree in pharmacy required (BS Pharm). Doctor of Pharmacy preferred (PharmD).
  • 2-5 years clinical pharmacy experience with 1-2 years in specific practce area.
  • Residency/fellowship in the field of specialty is desired and will count as 2 years’ experience.
  • Strong proficiency in computer literacy including but not limited to MS Office, Pharmacy clinical systems (Cerner), and Pyxis technologies.
  • Active Board Certification or ability to obtain within 12 months of hire. Other certifications that meet ASHP preceptor qualifications relevant to practice area may be considered. Emergency Medicine or Critical Care preferred.

Responsibilities

  • ER Clinical Pharmacist II within highly integrated and collaborative ER pharmacy program.
  • Participate in interdisciplinary rounds/meetings.
  • Profile orders for medications.
  • Monitor patient’s drug therapies.
  • Provide drug information.
  • Initiate/modify drug therapy based on medical staff approved policies and protocols including nutrition support and pharmacokinetics.
  • Supervise and direct supportive staff, precepts pharmacy students, and pharmacy trainees.
  • Provide information regarding medication therapy in both formal and informal settings to healthcare providers, residents, staff, patients, and families.
  • In the absence of the Director of Pharmacy, a Clinical Staff Pharmacist II may be required to assume the essential responsibilities and perform the duties of the Director of Pharmacy.
