Note: This role requires conducting in-home clinical assessment visits as part of the enrollment process. Candidates should be comfortable traveling to participant homes and engaging directly with older adults in a clinical and supportive capacity. Role Scope: We are looking for a mission driven Registered Nurse to join our team to lead initial nursing and home safety assessments of prospective participants. This individual will help to bring the PACE (Program of All-Inclusive Care for the Elderly) program to serve frail elders and disabled older adults in the Sacramento communities. The Enrollment Registered Nurse is a key player in introducing participants to and assessing them for our PACE program. As one of the first care team touchpoints, this RN will act as a liaison between the IDT (Interdisciplinary Team) and the Outreach Team to ensure a smooth enrollment process and continuity of care for prospective participants. This role will be one of the first care team touchpoints with potential PACE participants.

Requirements

  • Passion for delivering amazing coordinated care to frail elders and disabled older adults
  • Associates degree from accredited nursing school; BSN preferred
  • Active and unrestricted RN License in the state of California
  • Excellent organization, communications, written and verbal communication skills
  • Excellent documentation skills for CRM/EHR systems
  • A minimum of one year experience working with the frail or elderly.
  • Proof of medical clearance for communicable diseases, including a TB test.
  • Proof of all immunizations up to date.
  • Proof of current CPR/BLS certification or able to obtain certification within 30 days of employment
  • Proof of valid CA driver’s license, personal transportation, good driving record, and auto insurance as required by State law

Nice To Haves

  • Bilingual: Spanish strongly preferred
  • Experience working with Medicare population
  • Prior patient assessment experience in SNF or Nursing homes
  • 2+ years healthcare experience as a RN

Responsibilities

  • Collaborate with Outreach Specialist and IDT (e.g., Primary Care Physician, RN Care manager, Medical Social Worker, Home Care Coordinator, etc.) to ensure successful enrollment, and handoff that is clinically effective and a great experience for the participant / caregiver
  • Deliver an experience to potential participants and their caregivers that is welcoming, caring, and highly responsive to their needs and concerns
  • Conduct home visits to evaluate the prospective participant for potential eligibility for nursing home level of care
  • Conduct home safety evaluations of the prospective participant’s home
  • Participate in Enrollment Eligibility meetings to review Level of Care recommendations
  • Complete Level of Care (LOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCS.
  • Provide clinical recommendations for IDT consideration regarding enrollment appropriateness
  • Participate in Health and Safety enrollment decision meetings.
  • Maintain accurate documentation and record all evaluations in appropriate software
  • Participate in all PACE Audits by DHCS and CMS
  • Maintain knowledge and expertise in eligibility, enrollment, and program specification for the PACE Program
  • Support business development and census growth by participating and assisting with outreach programs as needed
  • Other duties as assigned
