About The Position

NAMI San Diego & Imperial Counties (National Alliance on Mental Illness) is a nonprofit organization that provides free education, support services, and advocacy to improve the quality of life of everyone affected by mental health conditions. Our Enhanced Care Manager (ECM) support individuals who are Medi-Cal members, served by managed care plans in San Diego County, and are part of one of the "populations of focus" identified by CalAIM: individuals experiencing or at risk of experiencing homelessness, individuals who have frequent avoidable visits to the ER/hospital, individuals with serious mental health conditions or substance use disorder needs, children/youth in CA children's services needing additional support, or children/youth presently in foster care or who were in foster care on their eighteenth birthday, individuals transitioning to the community following incarceration, adults at risk for long term "institutionalization," or individuals staying in skilled nursing facilities wanting to transition to the community. Additionally, we provide services to individuals who are incarcerated and nearing their time of release. Our ECMs provide an array of services to support the individual's health and well-being. This position is based in San Diego County; our ECMs work in the field in all areas of San Diego County, as well as in our offices.

Requirements

  • Minimum one year of experience providing case management, care management, peer support, housing navigation, or related support.
  • Minimum of one year of experience supporting unserved/underserved groups such as those described in the primary responsibilities/functions section.
  • Personal lived experience or with a family member or loved one with mental health conditions, substance use, homelessness, or any of the other underserved groups described in the primary responsibilities/functions section.
  • In-depth knowledge of San Diego County's health, behavioral health, benefits, resources, social services, and culture.
  • Intermediate to advanced proficiency with computer software (Microsoft Office: Outlook, Word, Excel) and web navigation.
  • Strong positive viewpoint towards individuals living with mental health conditions, substance use conditions, homelessness, recent history of justice involvement, and their families.
  • Able to communicate effectively, verbally and in writing.
  • Demonstrated ability to maintain confidentiality.
  • Exceptional interpersonal skills, positivity, attention to detail, flexibility, and enjoy working in an ever-changing environment.
  • Excellent planning, organizing, analyzing, problem-solving, and time management skills.
  • Ability to get along and work effectively with others.
  • Reliable and consistent attendance.
  • Must be willing and able to drive your personal car to transport clients, have a valid California Driver's License and comprehensive automobile insurance as required by law. Must have a satisfactory driving record.

Nice To Haves

  • Fluency in Spanish, Vietnamese, Tagalog, Korean, Arabic, Farsi, Chinese, or ASL, preferred.
  • Knowledge of and use of recovery language, preferred.

Responsibilities

  • Build rapport and provide culturally sensitive, gender affirming, and trauma-informed support to all individuals served.
  • Conduct a comprehensive assessment to determine needs, preferences, and goals.
  • Develop and implement a person-centered care plan that aligns with the individual's needs, preferences, and goals.
  • Refer and connect to needed services, community support, housing, and social services.
  • Provide enhanced care coordination and communicate with all providers who support the individual.
  • Provide comprehensive transitional care to individuals entering or exiting hospitals and related treatment facilities, as needed.
  • Promote health, including education and support to individuals and their families regarding healthy choices, health challenges, and available resources.
  • Use evidence-based practices such as motivational interviewing to engage and help the member manage their care.
  • Complete timely and accurate documentation of eligibility, consent, assessments, progress towards goals, and all communications with the individual, their family, and providers.
  • Conduct outreach and engagement (for example, attending community events, meetings and presentations with providers, agencies, and members of the community).
  • Other duties as assigned.
