About The Position

The Enhanced Care Management (ECM) Supervisor position provides administrative management and supervision to the Medical Division of Omni Family Health in regards to the ECM program. As a certified Medical Health Home, Omni Family Health (OFH) is required to provide health care services to a defined patient population and to achieve specific health outcomes.

Requirements

  • BA/BS degree in Healthcare, Business Management or related field from an accredited college/university required.
  • Three to Five years of progressive supervision and management in service delivery.
  • Dependable motorized transportation.

Nice To Haves

  • Previous patient centered medical home and NCQA knowledge a plus.
  • Bilingual in English/Spanish preferred but not required

Responsibilities

  • The ECM Supervisor serves as a key member and leader of the ECM Team and is responsible for the implementation and development of program.
  • The ECM Supervisor works collaboratively with the health care team members, patients, families and caregivers to ensure safe, appropriate, comprehensive planning and delivery of care.
  • The ECM Supervisor is responsible for management of operations of the care team.
  • The ECM Supervisor has responsibility for development of quality measures and reporting them to the CB-CME and the Managed Health Care plan.
  • This position supports the overall development and implementation of policies and procedures for the program.
  • The ECM Supervisor assists in marketing the program to stakeholders and services as an ambassador for Patient Centered Care.
  • The ECM Supervisor works with staff members, community agencies and consultants to resolve the needs and problems of the member through the development of enhanced care management programs.
  • Works to establish and maintain interpersonal relationships with both internal and external staff and other agencies.
  • Serves as a team leader in ensuring communication among all IDT members.
  • Has the ability to demonstrate knowledge and skills to facilitate programming appropriate to the needs of the patient populations.
  • The ECM Supervisor has demonstrated commitment to the program vision and patient centered cue from a motivational interviewing, informed and culturally sensitive perspective.
  • Understands the importance of IT and data deliverables, and works collaboratively with the MCP to ensure necessary data is received/sent.
  • Exhibits knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking.
  • Possesses excellent written and verbal communication with contract providers and internal staff to promote effective and timely coordination of care and dissemination of policies and procedures.
  • Performs day-to-day administrative tasks such as maintaining information files and processing paperwork related to clinical and programing needs.
  • HIPAA compliance - Responsible for maintaining abreast of and in compliance with all HIPAA regulations and requirements. Treats all member information confidential.
  • Compliance - Ensure compliance with all local, state and federal regulations.
  • QA/QI Participate in QA/QI activities md contribute towards the overall performance improvement of the organization.
  • IT - Required to learn and use the Electronic Health Record and Practice Electronic System and its components. As required by the job functions and highlighted in the Policies and Procedures. These components include NextGen, PMS, QSI and other electronic features, as they are developed and implemented, as applicable to work environment.
  • All employees will participate in Patient Centered Health Home Model at Omni Family Health.
