Hushabye Nursery is an Arizona nonprofit organization with a mission to ‘embrace substance-exposed babies and their caregivers with compassionate, evidence-based care that changes the course of their entire lives.’ The organization was founded by Tara Sundem, a neonatal nurse practitioner who witnessed firsthand the challenges of newborns exposed prenatally to opioids and those families trying to care for them. Hushabye Nursery’s innovative model offers a safe and inclusive space where mothers, family members and babies – from conception through childhood – can receive integrative care and therapeutic support that offers each child the best possible life outcomes. Programs will include prenatal and postpartum support groups, inpatient nursery services and outpatient therapies. POSITION OVERVIEW: The Engagement Case Manager at Hushabye Nursery is responsible for managing client engagement and admissions, building and maintaining relationships with community and medical partners, and ensuring effective coordination of care for families with substance use disorders (SUD) and opioid use disorders (OUD). This role works directly in partnership and reports to the Engagement Administrator. The engagement case manager conducts client assessments, manages insurance processes, provides compassionate support, and facilitates group education and support utilizing evidence-based and trauma-informed care. FULL TIME, EVENINGS and WEEKENDS Hours are Thursday-Sunday, 11:30am-10pm

Requirements

  • Experience working with Medication Assisted Treatment, Opiate Use Disorder, Substance Use Disorder and Department of Child Safety preferred.
  • Knowledge of healthcare management principles.
  • Experience working with transdisciplinary health teams.
  • Effective communication, organizational and time-management skills.
  • Background in health care, social work, mental health, and/or substance use.
  • Problem solving skills and ability to multi-task.
  • Must be passionate about treating families who are struggling with substance abuse.
  • Ability to help foster a trauma-informed, inclusive environment for all mothers, families, and support systems.
  • Current AZ DPS Level 1 fingerprint clearance
  • A high school diploma or GED is required, and a bachelor’s degree is preferred.
  • Must have a minimum of one-year work experience in the field of substance abuse/mental health.
  • Must have Behavioral Health Technician Certification or meet the qualifications for a Behavioral Health Technician.
  • Experience and knowledge working with EMR and billing systems.

Responsibilities

  • Act as the primary liaison for referrals, managing the admissions process from initial contact to program entry, including scheduling and facilitating consent forms. Screening assessments, strengths-based needs assessments, and treatment plans with measurable goals.
  • Assign clients to appropriate clinicians and case managers, facilitating enrollment into Hushabye Nursery programming.
  • Ensure timely and accurate documentation in compliance with HIPAA regulations.
  • Work directly in partnership with the Engagement Administrator to support with developing and maintaining relationships with community and medical partners, particularly those working with pregnant women and families with Substance Use Disorders (SUD) and Opioid Use Disorders (OUD).
  • Attend local and state collaborative meetings to enhance care coordination and support for clients as needed.
  • Serve as an advocate for clients, attending court, making hospital visits, and Department of Child Services (DCS) meetings as needed.
  • Manage patient financial responsibilities, including processing and recording payments according to company and insurance policies.
  • Utilize EMR and CRM systems to manage client engagement, track assessments, and update client records.
  • Maintain data accuracy within the CRM, ensuring client information is current and correct, and provide coaching to staff as needed.
  • Generate reports from the CRM to monitor and report on engagement conversions, program participation, and other relevant metrics.
  • Facilitate and support group education and support sessions using evidence-based and trauma-informed care approaches.
  • Address clients' recovery and parenting journeys through structured group interventions and educational programs.
  • Provide care in a supportive, compassionate, and non-judgmental manner, respecting clients' diversity and rights.
  • Assist clients in navigating their care and accessing needed resources.
  • Greet families, answer phones, and manage multi-line phone systems with strong communication skills.
  • Adhere to professional standards, state laws, and organizational policies, including reporting child abuse or neglect as required.
  • Complete required orientation, continuing education, and additional tasks as assigned by senior leadership.
  • Maintain flexible hours to accommodate evenings, weekends, and on-call coverage as needed.
  • Other duties as assigned.
