About The Position

The End-of-Life Care Consultant will play a vital role within P3 Health Partners, serving as a key connector between our healthcare providers, patients, and hospice services. This position will be central to our mission of improving end-of-life care and increasing appropriate hospice utilization. The consultant will work to ensure that patients receive the right care at the right time, with empathy and respect guiding every interaction.

Requirements

  • Bachelor's degree in healthcare, business, or related field preferred but not required
  • Minimum of 5 years of experience in hospice liaison, healthcare business development, or related roles
  • Proven track record of building and maintaining strong professional relationships in healthcare settings
  • Extensive knowledge of hospice care, end-of-life issues, and relevant healthcare regulations
  • Excellent communication skills, with the ability to discuss sensitive topics with empathy and clarity
  • Strong analytical and problem-solving skills, with the ability to use data to drive decision-making
  • Ability to work independently and collaboratively in a fast-paced, evolving environment
  • Proficiency in CRM systems and healthcare-related software
  • Possess reliable transportation, a valid driver's license, and current liability insurance; willingness to travel extensively within the assigned territory

Responsibilities

  • Serve as a key resource on hospice and end-of-life care, providing education to healthcare professionals, patients, and families. Build and maintain relationships with P3 providers, Preferred Hospice Partners, and key stakeholders to drive appropriate patient referrals and admissions.
  • Identify and engage potential hospice candidates through patient panel reviews and collaboration with healthcare teams. Conduct assessments to evaluate patient needs and address barriers to accessing care.
  • Manage relationships with hospice companies, coordinate clinic communication, data exchange, and collaborate on best practices. Assist hospice partners with clinic challenges and education needs. Serve as the primary point of contact for hospice partners, working to improve patient outcomes.
  • Facilitate smooth transitions from referral to admission, maintaining detailed documentation in the CRM system, including daily notes. Meet or exceed hospice utilization goals while monitoring hospice care quality and addressing concerns to ensure a positive experience.
  • Contribute to developing best practices in end-of-life care and support team objectives with additional tasks as needed.
