We are seeking a detail orientated and dependable Employment Case Manager to join our Community Employment Services team! Requirements

• Experience working with persons with varying abilities.
• Ability to work a flexible schedule according to the needs of the clients and employers.

Nice To Haves

• Bachelor degree in Human Services or a related field preferred.

Responsibilities

• Connect with community employers to secure paid work and volunteer opportunities for referred individuals.
• Research and recommend appropriate community employment sites; complete vocational assessments, site evaluations, and follow-up services.
• Conduct career exploration by identifying vocational opportunities and matching employer needs with client goals.
• Assist clients with job applications, interview preparation, job skill development, and education on workplace expectations (including ADA information).
• Teach and reinforce positive behaviors, professionalism, and appropriate social interactions following Clarity Care’s proactive philosophy.
• Provide on-site job support to both workers and employers.
• Maintain accurate case management records, including progress notes, contact logs, and required documentation.
• Serve as a backup Job Coach when needed.
