The Emergency Solutions Case Facilitator operates as a team member under the supervision of the Emergency Solutions Team Manager. Primary job duties include 24/7/365 on call for crisis intervention services and acting as community liaison between Northeastern Center and referral sources. On call services include telephone, face-to-face, and telehealth interactions with clients. Follow-up action on previous emergency calls and maintaining positive working relationships with referral sources (hospitals, nursing homes, etc.) are other essential tasks.
