About The Position

The Home Visitor in the home-based program option is responsible for education, case management, and family advocacy to families in an assigned caseload by providing and linking families to comprehensive services including home visits and socialization activities. This position works with typically developing children and children with disabilities. Because this position is one part of a large system of services provided to children and families, this position also serves as a social service referral person and shares responsibility for parent involvement in the program.

Requirements

  • At least 2 years’ experience working with children and families in a human service setting.
  • Demonstrated knowledge of principles and practices of adult education. Ability to assist parents in devising and implementing strategies which will enable the families to maintain or work towards a stable and self-sufficient lifestyle.
  • Capacity to problem solves, handle crises, and work with families and children of low-income backgrounds.
  • Ability to work sensitively with families of diverse ethnic, language and cultural backgrounds.
  • Willingness to work primarily in the homes of families residing in high-risk communities.
  • Ability to communicate effectively in both written and oral forms with all levels of management, both internal and external to the agency.
  • Valid Texas Driver’s License, good driving record and reliable vehicle.
  • Must have own transportation and be able to travel independently to the homes of participating families.
  • Satisfactory completion of American Red Cross First Aid and CPR for children and adults within 30 days of employment (maintain current certificate during employment).
  • Current Blood Borne Pathogens training certificate within 30 days of employment (maintain current certificate during employment).
  • COMPUTER PROFICIENCY: • Intermediate skill in use of Windows, Excel, and MS Word. Database management (PROMIS) experiences a plus.

  • (Early Head Start Home Visitor)-Child Development Associate (CDA)
  • Equivalent combination of education and experience will be considered.
  • LANGUAGE REQUIREMENTS:• Bilingual Spanish / English preferred but not required.

Responsibilities

  • Plan and implement a developmentally appropriate home-based program for pregnant women and children ages birth to five and their families. Follows a comprehensive Home Based Curriculum Guide
  • Conduct weekly 90-minute home visits to young children and their families
  • Facilitate planned and organized group socialization experiences twice a month.
  • Provide families with developmentally appropriate activities and information designed to enhance the child’s intellectual, social-emotional, motor and language abilities.
  • Assist pregnant women to access comprehensive prenatal and postpartum care, through referrals. (EHS)
  • Works with parent to develop an individual plan for the child to include goal setting based on identified needs and prescriptions for objectives and activities to meet established goals (outcomes).
  • Promote parental involvement in curriculum planning Policy Council, parent committees and other community groups.
  • Screens, observes and documents child observations in GOLD.
  • Ensure an appropriate written Transition Plan for all children, six months before their third birthday; provide parents with training and information and establish communication to facilitate and coordinate the transition.
