About The Position

Boone Memorial Health is seeking a motivated and community-focused individual to serve as an Education Assistant supporting Population Health and Community Wellness initiatives. This role focuses on health promotion, chronic disease education, and leading community wellness programs that help individuals prevent and better manage their health conditions through education, guidance, and resources. Job Summary The Education Assistant supports the Population Health Community Wellness Program by assisting with program coordination, data entry, community outreach, and instruction. This position plays a key role in connecting community members with wellness programs and resources and supporting the successful delivery of educational initiatives.

Requirements

Strong understanding of delivering and implementing community programs and hosting events
Demonstrated experience in education
Strong communication skills with the ability to collaborate effectively with community partners, community members, and patients
Community wellness program certifications such as NDPP, WWE, or HHA are required (training may be provided during onboarding)

Nice To Haves

Health coach certification and experience with motivational interviewing are preferred but not required

Responsibilities

Assist with Population Health Community Wellness Program functions
Lead and provide community wellness programs
Complete data entry related to community wellness programs
Scan, copy, and fax materials as delegated by the Department Director
Manage signage for events, including placement and removal
Assist with referrals to community wellness programs
Contact participants by phone, email, or mail as needed
Attend and assist with community events during and after regular work hours as scheduled
Obtain certifications required for class instruction, including NDPP and similar programs
Perform other duties as assigned by the Director of Population Health
