The Expanded Community Services (ECS) program is a community-based team that works with clients placed in Adult Family Homes (AFHs) throughout the community. Clients have both behavioral and medical issues with a history of lost placements. Clinicians are assigned to a region so that they are able to devote their time to working with clients and the AFH staff in a particular area. Clinicians on this team are in the community much of the time, setting their own daily schedules and agendas. In addition to working with individual clients there are also many opportunities to collaborate with facility staff, other providers, programs, systems and collaterals. Each ECS client has a state social worker who works in tandem with us. This program has an excellent success rate with the overall goal of helping our clients be successful in their placements and within their communities. We do this by connecting with our clients and their staff, helping to develop useful plans and tools that the client and staff can utilize to work together and promote a healthy, stable environment.
