About The Position

Under the direction of the Program Manager, the Program Lead provides support and coordination of programmatic activities in addition to delivering direct services. This position is responsible for supporting program operations, leading projects, and providing an additional level of support for ECM Case Managers.

Requirements

  • Ability to perform the duties described above.
  • One to years of supervising experience.
  • High School Diploma or Equivalent.
  • Three or more years of patient navigation or case management experience.
  • Two or more years’ experience working in a health care setting.
  • Proficient with Microsoft Office applications.
  • Experience working with Electronic Health Records.
  • This position requires frequent driving. A valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability policies.
  • Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.
  • Ability to communicate well with people of diverse cultural professional and experiential backgrounds.

Nice To Haves

  • Language fluency verbal and written, in one of the following languages: English, Spanish, Vietnamese, Mandarin, Tagalog, Farsi highly desired.

Responsibilities

  • Conducts outreach to engage and enroll eligible community members to the Cal AIM ECM Program.
  • Conducts outreach to engage and schedule new patients assigned to clinic
  • Prepares, assigns and tracks lists for team to contact patients for outreach.
  • Completes high-risk behavioral health and housing assessments with client to identify need areas.
  • Develops and maintains an individualized care plan for each client.
  • Track program activities/outcomes, and provide progress reports.
  • Supervise, evaluate, and provide performance feedback to program staff on a regular basis.
  • Provide and ensure continuing training/education for staff for on-going development.
  • Conducts regular team meetings.
  • Coordinates care for clients with a multidisciplinary clinical team and outside agencies.
  • Facilitates referral linkages, navigation, performs case management services, and supports securing community resources.
  • Assists clients with the development of social and community support systems.
  • Develops effective working relationships with agencies, organizations, and providers to advocate for clients.
  • Documents and maintains clinical records per appropriate standards and regulations.
  • Conducts outreach in diverse community settings including shelters, drop-in facilities, and residential settings.
  • Participates in clinical supervision, consultations, and training.
  • Provides translation and language support services as needed.
