About The Position

Under general supervision, the ECM Case Manager is responsible for engaging patients and assisting with access to community resources. The ECM Case Manager will assist providers with complex patients and offer general support when situations arise. A successful ECM Case Manager will meet individuals “where they are at” and provide non-judgmental, non-coercive services, and resources to individuals seeking help. In addition, the role will provide consultation, education, training, and support in their work with participants through utilization of harm reduction strategies. This job description is intended to identify some of the primary duties and responsibilities. Mayers Memorial Hospital District reserves the right to modify, supplement, delete or augment the duties and responsibilities specified in this position description, at MAYERS MEMORIAL HOSPITAL DISTRICT’S sole and absolute discretion.

Requirements

  • High School Diploma or GED (Bachelors preferred)
  • One to three months related experience and/or training
  • 1-2 years Skill in communicating with patients and health professionals
  • Professional behavior is demonstrated, both when on duty and as a representative of MMHD outside of normal work hours.
  • Ability to work with others, at all levels within the organization, and collaborate effectively.
  • Above-average interpersonal, problem-solving, and written and oral communication skills.
  • A positive working relationship with patients, visitors, and facility staff is demonstrated.
  • Organizational ability and time management is demonstrated.
  • Produces deliverable products on time, within budget with minimal direction.
  • Demonstrates the ability to compile and organize data using Microsoft Office Applications.

  • Maintains an organized set of records, upon request, for the functions that are performed within the scope of this job.
  • Communicates appropriately and clearly to directors, managers, and coworkers.
  • Performs all assigned tasks accurately, including proper spelling and grammar, reference materials, and minimal mathematical calculation.

Responsibilities

  • Assist patients in establishing self-management goals using assessments and screeners.
  • Meet with patients to review and complete documentation and ECM enrollment agreements at the request of the medical team.
  • Receive warm hand-offs from medical providers to assist patients with basic needs and health coaching.
  • Utilize the ACE’s screener data to determine what services a minor or their immediate family require to reduce the long-term effects of chronic stress.
  • Communicate with patients and the medical team when there is an adjustment in the care plan including a request to change provider.
  • Training – remain current on MHSA laws, partnership guidelines, and job improvement trainings.
  • Track patient appointments including outreach, scheduling new appointments and following up on missed appointments to assist in the coordination of integrated behavioral health.
  • Assist in outreach and data collection of patient care in order accurately track and record patient progress.
  • Help with specified grants as assigned.
