This temporary position will be assigned to the Department of Health and Human Services. This position is located at 1329 N. Brightleaf Blvd., Smithfield, NC 27577 As an Early Intervention Service Coordinator-the duties include working with families and providers to develop Functional Individualized Family Service Plans (IFSPs), work with multidisciplinary teams to determine families' needs for support and services, conduct home visits, coordinator and facilitate services and transition planning; and participate in the authorization and monitoring of early intervention services.
