About The Position

This role works closely with outreach, medical providers, behavioral health providers, and primary care teams to provide care coordination for our patients and their caregivers to improve their health and general well-being through the coordination of care and services available at Terry Reilly and in the community. This position works to improve quality, increase patient engagement by helping the patients address the barriers to access care to keep patient active in their care. This position requires the ability to investigate and resolve discrepancies, think critically and analytically, and communicate effectively.

Requirements

  • 1 year of experience in a Primary Care setting or with experience in programs for young children ranging from birth to five years. Can include the provision of training and technical assistance/process consultation to teachers/home visitors and/or content managers in early childhood programs.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Proven record of customer service excellence including good verbal and written communication skills both in person and virtually.
  • Ability to navigate multiple computer software programs including EHR, Microsoft Office products, scanner, calculator, and multi-line telephone.
  • Ability to work independently with minimal supervision and use sound judgment.
  • Well-organized, creative, flexible, open-minded and able to manage several tasks simultaneously.
  • Ability to handle in a professional manner demanding or difficult patients and clients.

Nice To Haves

  • Previous customer service experience in a healthcare environment
  • Previous experience in a social service setting with at risk populations
  • Previous experience with billing/scheduling and EHR systems including EPIC
  • Bilingual (English/Spanish)
  • Knowledge and experience with health promotion

Responsibilities

  • Assist Clients and patients in accessing community-based health related services
  • Assists patients with problem solving potential issues related to healthcare system, financial or social barriers connecting and referring patients to Patient Navigator
  • Answers questions regarding client appointments and testing.
  • Responsibilities are performed in a competent manner, within scope of practice and with minimal supervision.
  • Information provided to clinicians, staff, and community providers is clear, accurate, timely, and professional.
  • All referrals are addressed in a timely manner as defined in current protocols.
