About The Position

Center for Human Development (CHD) is currently seeking a Dual Licensed Family Nurse Practitioner (FNP) / Psychiatric Mental Health Nurse Practitioner (PMHNP) as we extend our continuum of service within the community. We will work with adults, children, and families, addressing a broad range of social and emotional needs using evidence-based models of care.    Position Summary: The CHD Outpatient Site, located in Orange, MA, is seeking a dedicated and experienced dual licensed FNP/PMHNP to provide integrated primary care and behavioral health services to individuals across their lifespan. This role is essential to advancing whole-person care in a community-based setting, addressing both physical and mental health needs with compassion and clinical excellence. 

Requirements

  • Active and unrestricted licensure as both a Family Nurse Practitioner (FNP) and Psychiatric Mental Health Nurse Practitioner (PMHNP). 
  • Current DEA registration and prescriptive authority in the state. 
  • Minimum of 3 years clinical experience in primary care and/or behavioral health settings. 
  • Proficiency in using electronic health records and telehealth platforms. 
  • Commitment to serving underserved populations and working in a team-based care model. 

Nice To Haves

  • Experience in integrated care models or Federally Qualified Health Centers (FQHCs). 
  • Familiarity with CMS guidelines, value-based care, and quality metrics. 
  • Bilingual or multilingual capabilities. 
  • Experience with trauma-informed care and culturally responsive practices. 

Responsibilities

  • Deliver comprehensive primary care services including preventive care, chronic disease management, and acute care for patients from pediatrics to geriatrics. 
  • Provide psychiatric evaluations, medication management, and therapeutic interventions for individuals with mental health conditions. 
  • Collaborate with interdisciplinary teams including medical providers, behavioral health clinicians, case managers, and community partners. 
  • Participate in care coordination, quality improvement initiatives, and population health strategies. 
  • Maintain accurate and timely documentation in the electronic health record (EHR). 
  • Support crisis intervention and stabilization efforts as needed. 
  • Engage in community outreach and education to promote health equity and access.
