About The Position

Serve in a case management role as part of the Brunswick Judicial Circuit Drug Court team. This position would be a part-time position located primarily in Wayne County. This position would report directly to the Drug Court Administrator and would work with all clinical and non-clinical staff to support case management activities as needed.

Requirements

  • Bachelor’s degree or work experience at a minimum of two years in a related field such as social work, psychology, criminal justice, human services, behavioral science, sociology, public health, or similar. An associate’s degree and related work experience of at least three years would be considered.
  • Proven working experience in case management, including as a case worker or manager or a related job field.
  • Excellent knowledge of case management principles, healthcare or human service management .
  • Previous experience with psychological aspects of care.
  • Effective communication skills (oral and written)
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Able to work independently as well as part of a team
  • Able to travel within Judicial Circuit

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered.
  • Complete case assignments, draft and implement service plans, review case progress and determine case closure.
  • Provides leadership in development and implementation of early intervention supports such as anger management, parenting and budgeting.
  • Assist clients to achieve wellness, stability, and autonomy through access to resources such as housing, employment and other support services.
  • Facilitate multiple care aspects (case coordination, information sharing, etc).
  • Assist clients with obtaining a GED as needed or other skilled training for employment stability.
  • Develop effective working relations and cooperate with clinical team throughout the entire case management process.
  • Assist with initial intake processes to determine client needs and prioritize them through development of effective service plans.
  • Take the extra mile and interact with clients to keep track of their progress and to ensure satisfaction by maintaining regular contact.
  • Record case information, complete accurately all necessary forms and produce statistical reports.
  • Ensure Casework is always updated to include all status changes, violations and sanctions.
  • Prepare the court reports for Wayne and Appling Counties prior to court.
  • Attend all court staffing and court hearings as scheduled.
  • Prepare and review all graduation packets with clients.
  • Submit to Probation for their review and approval.
  • Obtain booking photos and current photo for graduation.
  • Assist clinical team with preparing all quarterly reports.
  • Promote quality and cost-effective interventions and outcomes.
  • Adhere to professional standards as outlined by protocols, rules and regulations
