About The Position

The Donor & Patient Safety Coordinator ensures effective and ongoing compliance with OPTN policy by coordinating with operations staff to provide quality checks for ongoing case activity, assisting the Donor & Patient Safety Program Manager (DPSPM) with follow-up for patient safety related events and compliance reporting to OPTN, and assisting the DPSPM with management of donor/patient safety documentation and nonconformance management. This position has no formal supervisory responsibilities. Requirements

  • Familiarity with current good documentation practices, quality assurance principles and application of quality improvement tools.
  • Familiarity with medical terminology, electronic medical records, and ability to analyze complex medical records for risk assessments
  • Excellent written and verbal skills.
  • Multi-tasking, i.e., ability to function and complete work with competing priorities and expectations, and good problem-solving skills.

Nice To Haves

  • A Bachelor’s Degree is preferred
  • Documented training courses and/or certifications in Quality assurance are preferred.
  • 4 years of industry experience that is related to organ donation, healthcare and/or quality assurance is desired.
  • Previous experience in OPOs and/or tissue banking

Responsibilities

  • Collects and reviews donor test results post-recovery, and reports necessary results to internal and external stakeholders, including operations and LCNW Leadership, tissue partners, transplant partners, and regulatory agencies as required
  • Ensures timely and accurate communication to stakeholders, any relevant test results and other relevant information related to patient safety and the quality and safety of organs and tissues recovered by LifeCenter.
  • Responds to transplant center and tissue processor requests for additional information in a timely manner.
  • Assists the Donor & Patient Safety Program Manager (DPSPM) in providing initial and ongoing follow-up for patient safety related events, including sharing information with transplant centers, UNOS, and medical advisors.
  • Assists with investigations and implementation of corrective actions when a patient safety event is identified and creates and completes related effectiveness checks.
  • Coordinates with operations staff to perform real-time quality check of active organ cases requiring infectious disease consults, additional test follow-up, and evaluation of high-risk behaviors.
  • Serves as a liaison between clinical staff and the Quality & Regulatory Affairs staff for accurate and timely completion of Corrective and Preventive Actions (CAPAs), Root Cause Analyses (RCAs), and complaints related to donor/patient safety.
  • Serves as a resource to departmental management in monitoring and analyzing proposed changes to regulations and standards for impact to chart review policies and practices and OPTN reporting requirements.
  • Works with the DPSPM to ensure data reporting compliance for regulatory agencies and works with operations staff to reconcile errors and omissions.
  • Assists with recovery, packaging & labeling, and allocation audits as requested and provides concise and timely audit reports to Quality & Regulatory Affairs Leadership.
  • Receives, documents, and manages nonconformance reports in the electronic Quality Management System (eQMS).
  • Assists the DPSPM in timely investigation of non-conformance reports and implements recommended corrective/preventive action as necessary.
  • Assists in preparation for and participates in regulatory audits.
  • Performs preliminary review of critical donor information including but not limited to serology testing, declaration of death, authorization for donation, and medical and social history when donor and/or patient safety events are identified.
  • Works with other organizations (Hospitals, eye-banks, physician offices, Medical Examiner/Coroners) in requesting relevant donor information including autopsy, toxicology, serology testing results, medical examiner and coroner’s investigative reports, medical records, and results of donor testing
  • Creates and distributes Potential Disease Transmission Event reports to medical directors/medical advisors as requested by the DPSPM.
  • Performs data entry of donor and recovery information to the LifeCenter Electronic Donor Management System and other databases.
  • Participates in regular performance review, tracking workflow and generation of performance metric reports to ensure key performance indicator goals are met.
  • Ensures that donor information is handled in a secure manner.
  • Assists with the development of training materials regarding Electronic Donor Management System data entry, chart review and operational activities.
  • Assists with development and creation of controlled documentation (policies, procedures, and work instructions) as requested.
  • Assists with the development of assessments to evaluate employee competency and the effectiveness of staff training.
  • Works closely with stakeholders to ensure compliance with all LifeCenter SOPs, and regulatory agency requirements.
  • Works closely with the Quality and Regulatory Affairs team for periodic evaluation of Donor & Patient Safety Program activities to streamline workflow, increase productivity, and creatively problem-solve.
  • Performs other related duties as required by the needs of the company and the management team.
