Discharge Specialist - Patient Case Management
About The Position
The Discharge Specialist, under the direction of the RN Case Manager or Social Worker, is responsible for implementing discharge/transition planning services. Coordinates with the Case Manager or Social Worker, others on the interdisciplinary team, and external post discharge providers, to obtain services and supplies for patients and families in preparation for discharge/transfer. Requirements
- High School Diploma or Equivalent Required
- 1 Year Clinical - Discharge Planning Required
