About The Position

Mountain View Hospital is looking for a Discharge Planner join our team! JOB SUMMARY: Works in close collaboration with physician health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patients’ health problems. Prepares patient and family for safe discharge from acute care hospital setting.

Requirements

  • Education/Certification: Could be a graduate from an accredited school of nursing with a current license as an LPN or RN in the state of Idaho, with preferred experience in Case Management. Or, a Licensed Social Worker, bachelor’s degree (B.A.) in social service from four-year college or university; preferred experience in a healthcare facility.
  • Experience:3+ years of clinical nursing.
  • Equipment/Technology: Basic computer knowledge.
  • Language/Communication: Effective written and oral skills. Ability to comprehend and follow detailed instructions. The ability to effectively present information and respond to questions from groups of managers, patients, customers, and the pressure during difficult circumstances. Speaks clearly and persuasively in positive or negative situations listens and gets clarification.

Responsibilities

  • Coordinate the discharge plan and services with the patient, their families and disciplinary teams to ensure that a patient has an appropriate plan and services when ready to discharge.
  • Work with patients while obtaining maximum benefits from medical care by performing the following duties.
  • Reviews and validates physician orders are carried out, reports progress and unusual occurrences on patients to department supervisor.
  • Ensures cost effective and appropriate health care services are provided to all patients regardless of patients funding.
  • Collaborates with other departments in discharge planning.
  • Assists in assessing the physical, functional, social, psychological, environmental and financial needs of patients.
  • Assists with referrals to appropriate community resources and contributes to the assessment of patient’s support systems.
  • Acts as a patient advocate by assisting in acquiring appropriate medical, nursing or other health care in the home or clinic setting as needed upon discharge.
  • Maintains accurate patient records, ensuring patient confidentiality.
  • Contributes and assists in the performance of case management activities, and contributes to the assessing, planning, implementing, coordinating, monitoring and evaluating of the needs of patients for services both during and after hospitalization.
  • Assists the department head in the supervision of the utilization of resources to promote quality, cost-effective outcomes for the patient and the patient’s family.
