About The Position

UPMC Shadyside is hiring a Full-Time Discharge Planning Coordinator ! In this role, the Discharge Planning Coordinator will assist the Clinical Care Coordination and Discharge Planning Department with providing regulatory forms to patients, assisting with post-acute placement, and communicating with post-acute entities regarding the discharge. In addition to supporting the Discharge Planning team, seeking an individual with administrative support experience to assist the department, supervisors, and group of professionals. Experience with scheduling meetings, use of various applicable MS Suite products and business procedures preferred. Discharge Planning: Coordination of a patient's clinical care needs from either an inpatient hospitalization to outpatient; from a post-discharge facility to a home or assisted living facility; and/or coordination of resources to assist patients from an outpatient MD office. Includes, but is not limited to, insurance authorizations ( medication, transportation, alternate level of care), coordination of care to alternate levels of care ( skilled nursing homes, Inpatient rehab, home, including transportation), initiating and organizing hemodialysis, coordinating inpatient hospice, home hospice or skilled nursing with hospice; and obtaining information and connecting patients to appropriate outpatient regional resources.

Requirements

Responsibilities

Performs other duties as assigned.
Performs in accordance with system-wide competencies/behaviors.
Assists the Discharge Planning team with questions and troubleshooting that arise.
Gather and arrange the necessary materials to enable patient discharge.
Order supplies/equipment for patient-specific needs as identified by the clinical team.
Performs in accordance with system-wide competencies and complies with regulatory and HIPPA requirements.
Performs tasks delegated by Discharge Planning staff including clerical tasks such as copying or faxing as well as non-clinical tasks required to develop and execute a safe and effective discharge plan.
Provides forms, resources, and other information required as part of the discharge plan to the patient/caregivers as directed by the Discharge Planning staff.
Reports information in a clear, timely, and concise manner; Alerts Discharge Planning team of concerns related to plan of care and barriers to discharge.
Serves as a contact between patients and discharge planning staff.
Utilizes electronic medical records and technology programs to complete chart reviews, collect data, document progress, track communication, and other tasks as indicated.
