About The Position

Discharge Planner - Case Management FT/DAYS located at Corona Regional Medical Center in Corona, CA Reporting to the Case Management Director/Manager, this position provides evaluation and psychoeducation to patients and their families or significant others, to assist them in coping with psychosocial problems related to acute and chronic illness or injury. Assess patient current living environment and general medical/social needs. Provides intervention on order to maximize the patient's re-integration into the home and community. Able to provide competent care for patients throughout the lifespan. Integrates discharge planning process. Requirements

  • Age specific education and/or experience working with adult and geriatric behavioral health.
  • Excellent written and verbal communication.
  • Current BLS (CPR) Certificate American Heart Association.
  • Knowledge of Patient Care.
  • Medical Terminology.

  • One year clinical experience with dicharge planning and case management in a acute care setting.
