About The Position

Responsible for the delivery of professional nursing services which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of each individual patient/family. Provides care with a focus on discharge preparation and instruction, and coordinates discharge planning with other disciplines. Requirements

Graduate from an accredited nursing program.
Licensed Registered Nurse (LRN) - Single State - State Board of Nursing with eligibility to work in the state of MO.
Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)

Responsibilities

Performs patient assessment
Utilizes plan of care
Validates that patients have clear and concise information and understanding of discharge instructions
Completes accurate discharge medication reconciliation
Assesses patient/family learning needs
Utilizes interdepartmental collaboration according to patient needs to prepare for discharge
Provides staff education on discharge process
Ensures safety of patients, visitors, colleagues and self
May perform Staff Nurse duties as needed
Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.
