Valley Springs Behavioral Health Hospital offers compassionate, high-quality inpatient and outpatient mental health and co-occurring treatment for teens, adults, and seniors. We provide a full-range of evidence-based medical and clinical services in a state-of-the-art healing environment. Programs include inpatient mental health and co-occurring treatment, Partial Hospitalization Programs (PHP), and Intensive Outpatient Programs (IOP). The Behavioral Health Discharge Coordinator is responsible for coordinating aftercare services and supporting treatment planning to ensure continuity of care following inpatient discharge. This role collaborates closely with patients, families, providers, and the interdisciplinary treatment team to arrange follow‑up appointments, communicate discharge plans, and connect patients with appropriate community resources. The Discharge Coordinator supports crisis intervention efforts, maintains accurate and timely documentation, and ensures compliance with regulatory requirements including HIPAA, The Joint Commission, and applicable state and federal laws, while promoting a safe and supportive transition to ongoing care. Requirements

  • Bachelor's degree in social work, Counseling or equivalent required.
  • Basic Life Support certification and Handle with Care is required within 30 days of hire.
