The Discharge Coordinator/Case Manager RN performs a variety of duties centered around the discharge and case management of patients. Assesses patients for transitional care needs and barriers to discharge, communicates with the patient and family on their preferences, charting, and documentation of plan of care, coordinates plans for discharges involving community resources and alternate levels of care, facilitates successful adherence to the discharge plan, maintains high-quality, patient-centered, and cost-effective results, and complies with all regulatory requirements. The Discharge coordinator/ Case Manager has a pivotal role in providing a smooth transition to the patient's next level of care. This role requires the ability to utilize critical thinking, multitasking, and prioritization. An additional requirement is strong communication skills to promote collaborative relationships with the patients, their families, and the healthcare team.
