About The Position

Job Purpose or Objective(s): The Director of Healthy Aging oversees the Healthy Aging programs across the Choctaw Nation. You will establish and develop all aspects of the Healthy Aging program to provide our tribal elders with resources and opportunities to sustain a healthier lifestyle. You will report to the Executive Director of Clinical Services. PG 16 - Schedule: Monday through Friday 8:00am to 4:30pm | Weekly Earned Wage Access is an Option for this Position | Primary Tasks: Direct the daily operations of the Healthy Aging programs including health and wellness, case management, social services, behavioral health needs, and geriatric clinics to meet the needs of our tribal elders. Create and oversee an annual million-dollar budget, plan healthy aging programs. Establish and continue development, implementation, and continued evaluation of community-based programs and clinics directed towards tribal elders. Facilitate the development of policies and procedures to achieve the objective of maintaining our tribal elder’s health and independence; ensure that communication is effective regarding our policies with tribal elders, health care professionals, staff, and family members to establish a cohesive team. Develop marketing strategies by collaborating with our marketing team to ensure search engine visibility and email outreach, social media presence, and other referral channels for optimal service accessibility. Direct the design and preparation of media campaigns and information for community outreach including social media, web access, pamphlets and flyers; monitor analytics monthly. Submit and present monthly management and outcome reports including reports detailing referrals, assessments, visits, and interactions; complete grant administration reports bi-annually. Maintain liaison with other agencies involved with serving the elderly and further the vision of healthy aging. Perform other responsibilities as may be assigned.

Requirements

  • Bachelor’s degree in nursing or social services or; Equivalent combination of education and experience of six [6] years
  • Excellent oral and written communication skills
  • Strong interpersonal skills
  • Must be able to travel within the assigned service area
  • Experience with community resources for elderly clients and caregivers
  • Total of ten [10] years of experience in ambulatory care setting, community/preventative health nursing, or home health care
