A Director, Southern Bureau North Housing and Care Management Services (DSBHCM)) contributes to AHF by providing leadership and managerial oversight of Care Management programs and contracts. The DSBHCM is accountable to the Chief of Care Management for leading and end-to-end administrative and operational oversight of the Housing/Care Management teams within the Southern Bureau North that supports Care Management Division (CMD) and AHF goals for growth, retention of clients, seamless patient experience in the AHF healthcare system, as well as effective care management strategies that improve patient quality of life, stability in housing and compliance with all grant and contract deliverables and goals. This position requires ongoing subject matter expertise as it relates to Ryan White grant-funded contracts as well as Housing Opportunities for Persons with AIDS (HOPWA), Housing and Urban Development (HUD). The DSBHCM collaborates with the AHF Southern Bureau Regional team, the Leadership Team, and Healthcare Center staff to ensure that all staff understand operational processes, deliverables, and performance goals of the Care Management Division and Housing Programs. In this role, the DSBHCM will provide guidance and operational support to your multiple team members in multiple states, e.g., Texas, Mississippi, Tennessee, and growth sites/state as growth occurs. The DSBHCM monitors and analyzes productivity and manager and staff performance, and identify trends, best practices, and challenges. By conducting regular one-on-one meetings with managers and staff, as well as periodic site-specific oversight visits you will gain insight into best practice, successes, barriers and challenges faced at each site. With a solution-oriented focus, you will foster collaboration across AHF departments to ensure seamless service delivery and adherence to organizational standards. The DSBHCM holds to account and supports the housing programs by assuring ongoing education and training of housing managers, housing case managers as it relates to the grant, contract, federal and state regulatory requirements to assure compliance and achievement of goals and deliverables required. The DSBHCM works with staff to develop outreach and educational programs for clients referred to through the community housing continuum of care and are seeking housing stability and/or permanency to promote successful permanent housing. The DSBHCM also works with Ryan White managers and staff to hold them to account and assure knowledge of their individual grant goals, CMD principles of care management and deliverables required to execute the grants and effectively, efficiently and empathetically serve Ryan White clients. When a Housing or Ryan White care management grant becomes available in the region, the DSBHCM participates with the AHF team in providing data, information and structural response to the request for proposals (RFP) Upon award, the DSBHCM is responsible for leading CMD team in the implementation of care management and/or housing grants. The Director will support with building up the programs and maintaining all expectations to the highest standard. The DSBHCM works with partner agencies to coordinate care, remove barriers to housing, and provide targeted referrals for behavioral health, medical, financial, and social services. A DSBHCM enhances the reputation of AHF by providing consistent, field-based support to staff, patients, residents enrolled across all Ryan White and available housing programs. The DSBHCM oversees each program to ensure compliance with approved caseloads, delivery of required services, and completion of mandated housing monthly in-home visits. Through this oversight, the Director monitors the housing managers’ management of the housing case managers ability to assist clients in housing stability, identifying emerging client needs, and ensures timely referrals to appropriate resources. The role includes regular participation in coordination meetings with Ryan White/Housing staff and partner agencies to align care strategies and maintain adherence to jurisdiction Ryan White Care Management Guidance standards, contracts and Housing and HOPWA contractual and program standards of care. In addition to direct client support, the Director cultivates relationships with external service providers, identifying opportunities to establish formal partnerships through memorandums of understanding (MOUs) in the multiple jurisdictions and states. The DSBHCM will work as a part of a close-knit team to support care managers, housing staff, patients and residents in programs such as Ryan White, TBRA, PHP, STRMU, and HUD by accepting referrals, completing client enrollments, and developing individualized care plans. The DSBHCM will become familiar with Ryan White local guidance, Housing Forward’s intake, recertification, and exit processes, and will document all client encounters and actions taken on the client’s behalf. They will also contribute to a dynamic resource database and help ensure services are equitable, inclusive, and tailored to individual needs.
