For nearly 70 years, Stella Maris has been a leading nursing home, providing Baltimore-area residents with a comprehensive range of health and residential services. We are currently recruiting for a Social Worker Director to join our team. This is an exciting opportunity to join our mission driven organization to provide excellent care to our residents. Requirements

Licensed SW in the State of Maryland required
Degree in SW required
Minimum of 3 years LTC experience required

Nice To Haves

Prior supervisor experience preferred

Responsibilities

Responsible for planning, developing, organizing, implementing, evaluating and directing the operations of social service programs for the facility.
Promotes the mission, vision, and values of Stella Maris
Develop, plan and implement program goals, objectives, personnel, resources and equipment.
Develop and implement policies and procedures for identifying the medically-related social and emotional needs of the patients/residents
Assists in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (including collaboration with community agencies) and providing referrals to other community agencies when specialized assistance is required
Provides supervision to staff that meets regulatory compliance
Provide clinical consultation, oversight of case planning and care coordination.
Responsible for a caseload.
Direct crisis intervention efforts and intervene as required.
Provide staff management to include hiring, development, training, performance management and communication to ensure effective and efficient department operation.
Ensure procedures and workflow are consistent with organizational objectives, treatment protocols, regulations and standards of care.
Ensures regulatory compliance with MDS, BIMS, PHQ9, Care planning
