About The Position

Heather Hill Health Care Center is seeking a compassionate and experienced Director of Social Services to join our leadership team. The ideal candidate is a resident-focused professional who understands the unique needs of individuals in a skilled nursing setting and is passionate about ensuring quality of life, dignity, and emotional well-being for every resident. As the Director of Social Services, you will lead and coordinate social service programs that support residents and their families from admission through discharge, working closely with an interdisciplinary team to promote positive outcomes.

Requirements

• Bachelor’s Degree (BA or BS) in Social Work or a related field is REQUIRED
• Current Florida licensure as an LBSW, LCSW, or equivalent (required)
• Minimum of 2 years of experience in a skilled nursing facility or long-term care setting (required)
• Strong knowledge of federal and state regulations governing SNF social services
• Excellent communication, documentation, and leadership skills
• Compassionate, resident-centered approach with the ability to support families through transitions of care

Responsibilities

• Oversee all social service functions within the facility, including admissions, care planning, and discharge coordination
• Conduct assessments to identify residents’ psychosocial, emotional, and support needs
• Serve as an advocate for residents and families, ensuring individualized care and satisfaction
• Collaborate with nursing, therapy, and administrative teams to address resident goals and concerns
• Maintain documentation in compliance with state, federal, and company guidelines
• Provide leadership, training, and support to social services staff
