About The Position

The Director of Social Services oversees Community Health Resource Coordination, community impact relationships with partners, grant program reporting/management, and addresses systemic issues impacting community well-being.

Requirements

  • 5+ years of experience in public health setting or Bachelor's degree with experience/background in public health setting
  • 5 years of administrative experience, required
  • Comprehensive understanding of improvement science and quality improvement tools
  • Excellent and effective communication skills, both written and verbal
  • Strong analytical and interpersonal skills
  • Ability to perform data analysis and prepare and present reports for a variety of audiences
  • Ability and willingness to collaborate with all leadership and management positions within the organization as well as staff in all departments
  • Ability to prioritize and manage multiple simultaneous responsibilities
  • Ability and willingness to accept direction and follow established policies and procedures
  • Ability to work independently and be self-initiating, yet maintain self-awareness and recognize limitations, seeking assistance and opportunities for training and personal development as needed
  • Ability to interact with a high level of tact and diplomacy regarding sensitive matters
  • Ability to honor patient and staff confidentiality
  • English proficiency required

Responsibilities

  • Supervises and supports Community Health Resource Coordinators (levels I, II and III), throughout the organization. May supervise directly or through CHRC Lead(s).
  • Ensures community resource coordination/social services are seamlessly integrated throughout the work of Union on behalf of patients.
  • Collaboratively develops, implements and revises protocols, workflows and procedures in support of high reliability in processes.
  • Analyzes and reports on SDOH, health equity and social barriers to increase the knowledge, awareness and actions of Union to address whole health. Reports internally and externally.
  • Represents Union as a decision-maker on assigned community councils and coalitions with the intent to participate in collective impact and measurable outcomes for Lancaster/Lebanon communities.
  • Leads internal work related to SDOH and social needs.
  • Ensures all social services are measurable and appropriately documented for UDS (and other funder) reporting purposes.
  • Writes grant reports for funding agencies.
  • Participates in grant writing, when indicated.
  • Analyzes, validates and reports data internally and externally.
  • Communicates and relationship-builds with funders to maximize available resources for social services and collective impact.
  • Collaborates with all departments to close care gaps, maximize quality outcomes and coordinate care on behalf of patients with social needs that impact physical health.
  • Leads teams in identifying, prioritizing and developing action plans to respond to goals and emerging needs.
  • Ensures that each initiative us supported by adequate assessment, planning, implementation and evaluation.
  • Apply quality improvement cycle (PDSA) to departmental work.
  • Performs other work-related duties as assigned.
