Requirements

• A warm, approachable individual who leads with kindness and communicates with clarity.
• Bachelor's Degree in Social Work or related field (Strongly preferred)
• Experience in long-term care or healthcare social services
• Strong advocacy skills, emotional resilience and a geniune passion for serving older adults
• A collaborative spirit and a commitment to resident-centered care

Responsibilities

• Serve as a compassionate advocate for residents and families, helping them navigate transitions, challenges and care planning.
• Build trusting relationships through active listening, empathy and consistent follow-through.
• Support interdisciplinary care teams with insight and collaboration to ensure holistic, person-centered care.
• Guide residents and families through admissions, discharge planning and community resource connections.
• Maintain accurate documentation and uphold regulatory standards with integrity and professionalism.
