Responsibilities
- Meeting with residents and/or responsible parties to review policies on advanced directives, living wills, DNR status, residents rights and discharge planning. Implement documents as appropriate with residents and/or their responsible parties.
- Conduct and document social service evaluations including idenification of residents needs/issues and arrange for appropriate social support services.
- Provide assistance to residents and/or their responsible parties in obtaining financial services, as appropriate.
- Ensure that social servvices are coordinated with the other facility services.
