About The Position

The Lutheran Home, part of Lutheran Life Communities, is hiring a Director of Social Services to lead the Social Services department within our skilled nursing community. This leadership role focuses on supporting residents and families, coordinating care transitions, and ensuring high-quality psychosocial support for those we serve. Requirements

Minimum of three years of experience working in a Skilled Nursing Facility with 100+ beds
Strong leadership, communication, and organizational skills
Experience supporting residents and families in a healthcare or long-term care environment
Ability to collaborate effectively with interdisciplinary care teams

Responsibilities

Coordinate the full admission and transfer process for residents, ensuring smooth transitions and positive experiences
Lead and supervise the Social Services department while supporting and developing team members
Complete and maintain social service assessments, care plans, and MDS documentation
Provide support, guidance, and advocacy for residents and families to enhance well-being and quality of life
Serve as the abuse and neglect coordinator while ensuring compliance with state and federal regulations
Collaborate with interdisciplinary teams to support quality care and service excellence
Help ensure residents’ psychosocial needs are addressed and supported appropriately
