About The Position

The Director of Social Services & Life Enrichment serves as a corporate healthcare leader providing multi-facility oversight of Social Services and Activities programs across the Skilled Nursing Facility (SNF) division. This senior-level Healthcare Social Services Director ensures residents in long-term care and post-acute settings receive high-quality, compliant, and person-centered psychosocial services that improve quality of life, strengthen discharge planning outcomes, reduce rehospitalizations, and support successful care transitions across the continuum of care. As a subject-matter expert in SNF Social Services leadership and long-term care social work management, this role oversees CMS and state regulatory compliance, supports facility-level Licensed Social Workers and Activities Directors, and drives standardized programs that enhance interdisciplinary collaboration, improve survey readiness, and improve quality metrics. This position is ideal for an experienced Licensed Social Worker with regional or multi-facility Skilled Nursing leadership experience and a passion for regulatory excellence, performance improvement, and resident-centered engagement programming.

Requirements

  • Bachelor’s Degree in Social Work or related Human Services field (required)
  • Active Social Work license (required)
  • Minimum 5 years of experience in Skilled Nursing Facilities (SNF), Long-Term Care, and/or Post-Acute Healthcare
  • Minimum 5 years of multi-facility or regional leadership overseeing Social Services programs
  • Demonstrated expertise in: Psychosocial assessments, Discharge planning and care transitions, Interdisciplinary care planning, CMS, state, and federal regulatory compliance, Survey readiness and quality assurance
  • Experience developing engagement programming for memory care and palliative care units
  • Strong analytical skills with ability to evaluate trends, improve engagement rates, and support performance improvement initiatives
  • Excellent written and verbal communication skills

Responsibilities

  • Provide strategic, multi-facility oversight of Social Services and Life Enrichment departments across SNFs.
  • Develop and standardize programs aligned with CMS, state, and federal regulations to ensure survey readiness and compliance.
  • Lead multi-site quality assurance reviews and participate in survey validation processes.
  • Monitor psychosocial documentation, MDS-related care planning, and regulatory compliance metrics.
  • Educate teams on best practices in discharge planning and the full continuum of care, including home health, hospice, rehabilitation, and managed Medicare.
  • Partner with facility leadership to improve SNF discharge efficiency, reduce avoidable rehospitalizations, and strengthen post-acute referral networks.
  • Analyze utilization and referral metrics and implement targeted performance improvement plans.
  • Drive patient retention and appropriate placement within ancillary and post-acute service lines.
  • Ensure accurate and compliant completion of psychosocial assessments, social histories, and individualized care plans.
  • Support complex cases involving behavioral health, crisis intervention, and family dynamics.
  • Promote resident rights, dignity, trauma-informed care practices, and psychosocial well-being aligned with organizational values.
  • Oversee comprehensive Activities and Life Enrichment programs promoting social engagement, cognitive stimulation, and purposeful living.
  • Guide Activities Directors on calendar planning, documentation standards, staffing models, and volunteer management.
  • Support evidence-based programming for memory care and palliative care populations.
  • Ensure programs reflect cultural, spiritual, cognitive, and physical diversity of the resident population.
  • Monitor participation rates, documentation compliance, service line referrals, and departmental KPIs.
  • Support QAPI initiatives tied to psychosocial outcomes, quality of life measures, and CMS performance indicators.
  • Conduct regular facility visits to audit program effectiveness, compliance, and engagement outcomes.
  • Assist leadership in resolving patient complaints through compliance and ethics processes.
  • Lead onboarding and ongoing training for Social Workers and Life Enrichment Directors.
  • Facilitate annual and biannual leadership summits to drive standardization and best practices.
  • Maintain current knowledge of long-term care regulations, CMS updates, and healthcare social work standards.
  • Serve on corporate and interdisciplinary committees as assigned.
