Ensures the palliative care program runs smoothly and organizes staffing resources to meet patient needs and ensures caseloads are equitably assigned; ensures staffing is within budget. Supervises the Sincera Program Manager to ensure administrative and scheduling functions are optimal and support high quality palliative care. Works with the Sincera Program Manager and Finance team to ensure processes are in place that support maximum reimbursement and collections. Provides supervision of assigned staff and holds employees accountable to established agency performance standards. Inspects the work of team members to make sure that quality and performance standards are upheld. This includes but is not limited to: going on visits, reviewing productivity reports and clinical documentation. Works with the Chief Medical Officer on the development and revision of palliative care clinical policies and procedures as needed. Ensures annual audit of documentation is completed for each nurse practitioner. Leads by example and upholds the culture, standards, and values of the agency. Provides to team members assistance with problem solving to ensure the needs of patients and families are met. Provides timely performance feedback to employees and conducts annual performance evaluations on time. Works closely with HIM to monitor physician documentation and billing to ensure requirements are met in conjunction with the Director of Palliative Care. Ensures physicians complete professional credentialing, licensing and paperwork required for malpractice insurance coverage. Assists with the development and implementation of change processes to ensure employees are able to function at the highest level to be efficient and productive. As a leader in the organization, has responsibility to help the agency achieve its short-term and long-term strategic goals. Conducts monthly staff meetings and ensures the effective and positive communication of agency information and goals. Participates in the interview and selection process of new team members, as well as the orientation and training of new team members. Maintains open communication with Human Resources regarding employee performance issues; ensures documentation of employee performance coaching and counseling. Maintains knowledge and understanding of Medicare, Medicaid, Joint Commission, state and agency standards/regulations, and ensures the team’s compliance with these standards/regulations. Collects and organizes palliative care key performance indicators. Organizes and attends IDT meetings. Responsible for ensuring self and staff learn and utilize technology made available by the agency to increase efficiency, communication, and performance of duties. Works with the Admissions Team Leader to ensure timely transfers of palliative care patients to hospice. Participates in the agency’s Quality Improvement program and ensures the successful implementation of performance improvement initiatives. Attends required meetings and completes all mandatory training. Holds in strictest confidence all patient information and discloses information only to persons authorized by Hospice.
