About The Position

Director of Nursing & Clinical Operations (DNCO) – Hospice Full-Time | Home & Facility-Based Care Location: Arlington, TX + Service Area (DFW Metroplex) Lead from the Field. Build with Purpose. At Hospice Care Partners, we are not looking for a Director of Nursing who sits behind a desk. We are looking for a hands-on clinical leader who is visible in the field—supporting nurses, strengthening facility relationships, and ensuring that patient care is delivered at the highest level. Role Overview The Director of Nursing & Clinical Operations (DNCO) is a Registered Nurse responsible for the overall leadership, performance, and quality of clinical services across both home and facility-based care. This is a field-engaged leadership role , combining: Clinical oversight Team development Operational execution Relationship management You will lead a multidisciplinary team including: RN Case Managers Hospice Aides Social Workers Chaplain/Bereavement Coordinators Volunteers You may also maintain a limited caseload as needed. Requirements

  • Current, unencumbered Texas RN License
  • 3+ years hospice case management experience
  • 1+ years of hospice leadership/management experience
  • Reliable transportation with current insurance
  • Current CPR certification
  • Current TB test (negative) or equivalent
  • Ability to travel throughout service area
  • You take ownership of clinical outcomes and team performance
  • You are visible, engaged, and present in the field
  • You hold people accountable—consistently and professionally
  • You communicate clearly and proactively
  • You address issues early instead of allowing them to escalate

Nice To Haves

  • Experience scaling or growing a hospice program
  • Strong QAPI and clinical performance improvement experience
  • Experience leading interdisciplinary teams
  • Familiarity with Consolo EMR

Responsibilities

  • Lead and manage all clinical operations across home and facility settings
  • Be present and visible in the field : Shadow RN visits Support staff in real-time patient care situations Engage directly with facility partners and referral sources
  • Set and enforce standards for communication, accountability, and care delivery
  • Ensure appropriate staffing, productivity, and case distribution
  • Coach, develop, and hold clinical staff accountable to expectations
  • Address performance issues early and directly
  • Coordinate care across interdisciplinary teams, patients, families, and referral partners
  • Monitor, analyze, and act on QAPI data to improve outcomes
  • Ensure compliance with hospice regulations, documentation, and best practices
  • Strengthen clinical performance in key facility partnerships (e.g., nursing homes)
